This study aims to investigate the effect of the combination of NEXIUM and the combination of the anti-diarrhoeal drug esomeprazole (NEXIUM) on sperm motility, morphology, and semen quality in normal men with persistent idiopathic infertility.
BACKGROUND:Sperm motility is the key indicator of fertility in male and female infertility.
Esomeprazole (NEXIUM), a proton pump inhibitor (PPI), has been used as a first-line treatment for infertility in women and men, especially when unexplained infertility is present.
The purpose of this study was to evaluate the effect of esomeprazole (NEXIUM) on sperm motility, morphology, and semen quality in normal men with persistent idiopathic infertility.
SOLvENTS AND METHODS:Thirty-six patients with persistent idiopathic infertility who received treatment for infertility due to idiopathic infertility and/or male factor infertility (n = 28) and 14 patients who received treatment for unexplained infertility (n = 24) were enrolled. The patients were randomly divided into three groups. Group I received the combination of NEXIUM (esomeprazole + esomeprazole) and esomeprazole (esomeprazole + esomeprazole) (20 mg/day) at the start of treatment and group II received the combination of NEXIUM (esomeprazole + esomeprazole) and esomeprazole (esomeprazole + esomeprazole) (20 mg/day) at the end of the study (2 h before treatment), and group III received NEXIUM (esomeprazole + esomeprazole) and esomeprazole (esomeprazole + esomeprazole) (20 mg/day) at the end of the study. Semen volume was measured in the same groups by an abdominal sonography. Semen analysis was performed by a local anesthetic using an ointment. Blood samples were obtained and analyzed for serum total testosterone and free testosterone, sperm motility, and morphology. The patients were followed up every 3 months for a minimum of 6 months. Blood samples were collected in order to evaluate the effects of the combined treatment on sperm motility, morphology, and semen quality in normal men with persistent idiopathic infertility.
RESULTS:The results of the study were statistically significant in all three groups (P < 0.05). The mean values for the total and sperm motility were significantly lower in group II (P < 0.05), and those in group III (P < 0.01) were significantly lower in both groups (P < 0.05). Semen volume was significantly lower in group II (P < 0.05) and group III (P < 0.01) at both the beginning and end of treatment. However, the sperm motility, morphology, and semen quality were not significantly different in all groups. The mean values of total testosterone, free testosterone, sperm motility, and morphology in group II and group III were significantly lower in all three groups at both the beginning and end of the study. The mean values of free testosterone, sperm motility, and morphology in group II were significantly lower at the beginning of the study (P < 0.05) and those in group III at the end of the study. The mean values of the total sperm motility and the free testosterone in group II and group III were significantly lower at the beginning of the study (P < 0.05) and those in group III at the end of the study.
CONCLUSIONS:The combined treatment of esomeprazole (NEXIUM) and the combination of the anti-diarrhoeal drug esomeprazole (NEXIUM) with the combination of the proton pump inhibitor esomeprazole (NEXIUM) at the end of the study is associated with a decrease in sperm motility and morphology.
